Exercise bike. Wind sprints. 5 minutes warm-up. 30 seconds all out (higher resistance), 1 minute rest, 30 seconds all out (higher resistance), 1 minute rest. As long as you can keep this up.
I do this twice a week for 20 minutes; have been for the last 6 months. My resting heart rate is now 50, and I have no trouble running over a mile at a fast (~5:45) pace barely breaking a heavy sweat. This is from someone who never ran in his life. Trust me, wind sprints work.
