Injury during 1-rep maximum attempts is usually caused by some manner of form inconsistency. Also, if your connective tissues are being abused in your program, and you attempt a 1-rm, injury can result.
There's not really anything inherently wrong with pushing a max. In other words, the max is rarely the problem itself. Usually, the lifter is not wise in his methods. The man tends to break down before the movement.
