I have been playing for almost a year now... Im decent, but i need something that will give me that edge. I have really wide fingers and small hands so i cant reach across more than 3 frets really easily and causes problem hitting some chords. Do you have any suggestions to over come this problem?

Also i have problems transitioning from chord to chord sometimes, what will help with that?

To answer your Question Practice,And then some. I have small hands also The Way i used to practice especially if no one else was available ,was to play along with an album. What type of guitar & amp ya got?

i have the Martin Carthy limitedd edition signature guitar, and mahogany dreadnought 12 string.

As for electric goes i got a fender american deluxe fat strat

amp is hot rod deville 410

Im not much of an electric player though... Its easier, but i like the way the acoustic sound 10x better

As radar said, practice is the way. I don´t think anyone can teach how to play, you can only learn and try to make the best of the situation.
Try cheating if you can´t reach around the fretboard, sometimes it sounds better when you cheat :)

I learned to play by getting tabs from OLGA (RIP) on my favourite musicians and just tried to keep up with the songs.

I got an cheap Yamaha electric guitar but it sounds like I want it to, a takamine/Jasmine 6string accoustic and a LANEY TubeFusion800 amp with a homebuilt 4*12 with eminence PA-speakers. The sound is alot deeper and with more base and I can´t use it on stage but it sounds terrific at home and I can play the pants of my neighbours.
I also have a 1*12 combo from Hagstrom from the late 50s.

I started from tabs way back in the early 80's and progressed from there.

Just do a search for guitar lessons in any search engine. There is a huge amount of info out there.

Joe Satriani has an excellent exercise for chord transitions.......do a search at Guitar One for Joe.

I've been playing for 15 years.
I'm running a Jackson RR-4 and an ESP LTD H-300 into a Peavey 5150 half stack at the moment.

I love the mello sound of a tube amp- on stage i use a Peavy Duece hooked to a Peavy musician cabinet 6 12" woofers followed by a rockman and for the 60-70s sound corporated with a Big Muff Box,Dean Markly light guage strings, specially made Left handed Les Paul Gibson. During Peactice i play thru a Peavy Backstage Amp!
