I breastfed for over a year and wore a JJ cup bra. Before that I was a DD/DDD. After I quit nursing, lost weight, started training, I was down to a small D and sagged horribly. I had no choice but to get a lift and since I was going in for that I figured I'd get implants to bring me back up to close to what I used to be as a D or DD. I went with 450's, round high profile, overfilled, under the muscle and am pretty happy with them. Size is perfect, I'm a 36D in most bras. I've had complications with infection causing some scaring in areas and even though the surgery was done last April, I'm still dealing with infection from time to time. My body is rejecting the sutures, so I'm getting the lift portion redone in Feb. to get rid of the scars and to get the lift a bit higher.
Most women don't realize this, but there are two ways of going under the muscle...partial or completely under the muscle. I went completely under because I don't want to see the bags at all and I don't want to risk them dropping which would cause my boobs to sag yet again. Here's a site with info. detailing overs, partially unders, and completely unders:

Aside from risking future sagging, another reason I went under the muscle was to ensure clear mamograms in the future. If you go with overs, they can only do an ultra sound which isn't nearly as effective in detecting cancer. I'm almost 38 and things like this are very important to me.
I haven't trained chest much at all since the augmentation. When I do it simply hurts. My left side has some capsular contracture from the infection so with the new surgery the doc. will have to cut back into the muscle to correct it. Even doing skull crushers on the smith machine is pretty painful. Hopefully I'll be training chest again after this is fixed.
The anchor scar isn't bad. The line is very thin and if you tan it's hardly noticible. Just stick in some red lightbulbs in your bedroom and your guy won't even see them after a few months. LOL The color of the scar will be fairly dark for about 9 months post op, but by a year afterwards, it's very faint. The stitching is done on the inside so it's litterally a pencil thin line. Personally I'd rather have nice perky boobs with a little line on them than saggy boobs.
Going under the muscle also helps keep the breasts feeling natural especially if you have a decent amount of breast tissue beforehand. Mine aren't spread wide apart either. During the first few months post-op you need to massage them into place. It's not a fun massaging...it's more like pushing the implants under the muscle in towards your cleavage, but doing this religiously helps loosen the muscle so that the implants move into the right place.
The pain right after surgery really sucks with under the muscle implants. It's like...imagine having a softball shoved under your pec and the muscle being stretched tightly over it. Thank goodness for vicodin! I remember waking up in the middle of the night when the pain meds had worn off, barely able to get into a sitting position, then trying to open the pill container...I quickly learned not to tighten it all the way because I could barely push and twist that damn top to open it! You have no idea how many daily movements use your pec muscles! I also found that using a step stool for cooking helped a lot. I'm short. LOL
You won't be able to wear a regular bra for about 6 weeks post-op. I found my favorite bras at Target, sports bras that zip up the front. I wore those or just tank tops with shelf bras most of the time.
Nipple sensitivity...at first I had one nipple that was overly sensitive to the point where just my shirt brushing against the nipple hurt and the other was almost completely numb. Now, 9 months later, they're both just a little more sensitive than they were before.
OK...I've yacked enough here. I'll just add a few links and wish you all luck!
