I've been w/my GF for 3 years as well, and have have only used a condom if she forgot her pill.
She hasn't missed it but twice. And the 2 times she missed it I used a condom for a straight month afterwards. so Out of the 36 months of sex, I have only used a condom for 2 of those months. Those pills are extremely effective (even more so then a condom) but if you combine them, then well its VERY effective. The prob is that Birth Control and its potency is effected by a lot of little factors. My GF takes it everyday at about the same time (Give or take an hour). If your off by more then an hour then thats when the effectiveness starts plummeting. Also if a girl drinks, does drugs, or takes too much of certain vitamins/supplements it could kill its effectiveness even more. Even smoking can decrease the effectiveness. Add to that girls not telling their Boyfriends about 'forgetting' and thats where you get the girls, who claim to have been on the pill, getting knocked up.
Its safe, but you have to be open, communicative, and responsible in the same sense. She tells me when she misses a pill, and we are both very careful. and when she misses its a month of using a condom. She doesn't take any drugs or meds nor does she drink or smoke. So I'm 99.999% safe. I even contacted several Birth Control manufacturers and spoken with doctors who say the pill is 100% effective if taken properly. THey just can't claim 100% because of legal reasonings.
I could be wrong, but I've done a lot of research. The condom of course will help, but more or less the pill does the job for you, its really just an extra 'piece of mind' when you use the condom, but not really necessary if she is doing is right and her body chemistry is screwed around with.
