On my max effort days, the plan is still to get every rep. If I fail, it was a mistake.

Also, I am training around some rather impressive injuries sustained in an accident, so I am far more careful than I used to be.

Allow me to clarify....I NEVER go to failure on any of the assistance work, as I have stated. On the Max Effort Days...I plan to make every rep. I dont just go until I miss..as has been said...that would train me to get used to missing lifts. I just meant that I am not afraid to try a heavier weight because I might fail. If I have a choice between getting a 5 lb PR that I know I can make....or attempting a 10lb PR that I might miss...I will choose the heavier weight. If I do happen to miss the heavier weight...I can go back and get the lighter weight which is still a PR.
 
one the big three i go to failure on a few sets

today i went 1 rep on deads for 210lbs

up 15 from last week woo hoo
 
Failure makes it sound so negative!

On assistance exercises....i try and steer clear of failure.
Because i train the same assitance (or similiar exercise) 72 hours later.....i want to be fully recovered.
when you drop a couple reps short of failure, the next training session feels much more rewarding because its easier to progress and its a bit harder to get stale.

I dont know if its the best thing to do.....but if i fail a max effort lift......say a deadlift, i will not drop the the weight....and struggle for as long as i can hold onto it. To me that is a good way to overload, but that is just my opinion.
