Larger muscle groups (back, chest) get three different excercises. Smaller ones (arms, shoulders) get two. Legs get as many as five.
 
I agree except for legs.

What decent exercises past squats, extensions and leg presses can you do that are effective?

I'm gonna say 3 exercises for all large parts [chest, back, thighs] and 1-2 for smaller ones.

That depends upon what you consider "effective." I use machines for adductor, abductor, and both prone and seated curls.
 
Chest and Back I use 3 exr at 9-10 sets and everything else i do 2 exr about 6-7 sets.

Works Great for me :)
 
Ah my fault, i confused myself with 'legs' and 'quads' :)

yeah, if we're talking how many sets for thighs i'll say 4.
