Weights are all over the place. For example you do more on leg extensions than leg press. And why are you doing single leg leg press?
 
I follow the training schedule my trainer has written ... do you think I need to change things around?
You don't need to pay for a trainer. You can get bigger and stronger by taking our advice. You should be going much heavier. My wife lifts heavier and she is in her mid 50's and only 5'2".

An example would be bb bent over rows. You should be able to do sets of 8-10 with your BW.
 
a typical trainer at a gym just read a couple books and passed the test that's about it
