Would you be asking this question if you had an employee that wasn't meeting expectations? Your doctor works for you, you don't work for him. Yes, his pay is a PITA, and he's a well educated (in theory) specialist employee, but he's still an employee. Firing your doctor isn't much different than firing an under-performing employee.
FWIW, you don't actually have to notify his office. Just notify your insurance company and then sign a release of information form for your new doctor to request and receive your entire medical record from your current PCP.
