Advice when it comes to avoiding soreness

I've been out of the gym for a couple months now. I've come back before after lengthy times away and I'm usually super sore. I'm wondering what you have for advice when it comes to avoiding that soreness you get when you first come back. Any advice for me on what exercises I should avoid and should I cut back on the sets and reps that I'm doing?
