avoid the issue. they'll find out eventually if its an intimate situation.

I'm sure he paid for something, why didnt you catch a glance at a CC with his name on it or something.

well if he calls or you call him dont say his name say things like hi whats going on...if you dont intend on going with him again dont worry its ok every one forgets names.

I am HORRIBLE at remembering names... but, if I feel a real need to remember it, I will. Sad, but true. I'm constantly trying to work on that.

I just ask with an apology/self deprocating disclaimer in front. "I'm so sorry I'm such an idiot with names and I've forgotten yours."
