Depot Injection
The latest addition to the spectrum of testosterone preparations available to patients in the Western world is testosterone undecanoate in castor oil for intramuscular injection (Nebido®). This testosterone ester had been used in oral capsules since the 1970s (see above), but had obviously already been applied as intramuscular injections in China for several years,[59] when the author came across it at an exhibition accompanying an Andrology Symposium in Beijing in 1993. Samples brought to Germany were first injected into monkeys and showed a surprisingly long half-life.[60] The long half-life could be confirmed (21 days) in volunteering hypogonadal men who also showed serum levels in the normal range.[61]
Surprisingly, it was difficult to interest a company in this fascinating preparation. Finally, Michael Oettel at Jenapharm, then still an independent firm, recognized the potential of this testosterone ester. The vehicle was changed from Chinese teaseed oil to castor oil and, thus Europeanized, the preparation was developed for clinical use.[61-64] When compared to testosterone enanthate, serum testosterone levels remain in the normal range for long periods of time.[64] Some patients have now been followed for up to 8 years, were well substituted and show no serious side-effects.[65] Advertised as a 'men's quarterly', it requires injections of 1000 mg only every 12 (1014) weeks, following a loading dose at 6 weeks. Serum testosterone levels remain within the normal range. Adjustments of injection intervals may be scheduled according to serum testosterone levels determined from serum samples taken just before the next injection. The injection volume of 4 ml requires a deep and slow intragluteal injection, carefully avoiding the intravenous route.
In addition to its use in hypogonadism, testosterone undecanoate has become a valuable asset in male contraception trials and, as such, has outlived testosterone enanthate and buciclate. Testosterone undecanoate has the potential to be combined with norethisterone enanthate (NETE) into one contraceptive regimen[66-68] and, in combination with subcutaneous etonorgestrel implants, it is currently being tested in Europe by Schering and Organon in multicentre trials for male contraception.[69] At the same time the Chinese testosterone undecanoate preparation continues to be used for substitution therapy in China and is being tested in male contraceptive trials. Since this preparation in teaseed oil has a shorter half-life than its European variant in castor oil, it needs to be injected more frequently than Nebido®. However, a trial involving 308 couples in China, using monthly injections of 500 mg testosterone undecanoate, resulted in 97% of men showing azoospermia and complete contraceptive protection in these men.[70] Large-scale studies are currently being performed in China.
