Always start as low as possible and titrate up from there. You can even start at 0.5mg and increase by 0.5mg/week until you reach your threshold. 4mg/week is the highest that almost anyone needs (on EF at least).

If you keep getting nausea issues, then you need to lower the dose immediately or stop using it. The reason is that it has a super long half-life. It's not one of those things where you're going to get it out of your system overnight.
