Losing form on squats, help.

Hips need strengthening. . . start performing wide stance low box squats. Also include some hip abductor and adductor work. This is not uncommon. . .when i get really heavy this will sometimes happen to me.

Your left leg is weaker than your right

time to add a 2-3 sets of 10 single leg work after your main worksets. Just do a Bulgarian squat or stepups.
 
well you need to work both legs, but do an extra set for the left one.

basicly a split squat where the rear leg is elevated on a bench.
