Keep Pulling My Trap - Help!!!

I keep pulling the shit out of my trap on my left shoulder. It only ever happens to my left shoulder right in the middle of my trap between my neck and mid shoulder. The only thing I can think of that I'm doing right now in my current routine that could be affecting that muscle is tricep pull downs using a rope grip. But I always make sure my form is good and my arms are tight to my sides only letting my forearms control the motion. And I'm not jerking the weight. So I don't really get it. I pull this muscle like once every two months and it hurts like shit for days! I can't turn my neck and am in constant throbbing pain.

Since this only happens on my left side and never my right side I'm guessing I have some weak spot on that muscle or some spot that easily gets reinjured. How do I fix this? What can I do to strengthen that side so this stops happening?

I generally do shrugs for traps, but not this current routine, but have been doing pull overs which should hit traps some. Anyway, I need some advice. I've been working out regularly for 5 years and in and out of the gym for 15. So I realize you're going to get injuries, but this needs to stop. I'm sick of feeling like someone is stabbing me with a knife every two months.
 
That's definitely good advice and I have used a massage roller tube for deep tissue the last several days. It seemed to loosen up this evening pretty good.
But really what I'm most concerned about is figuring out how I've been hurting it to begin with or figuring out how to strengthen that area so it stops happening. It's seriously awful.
 
by any chance do you use a mixed grip (one hand pronated, the other hand supinated) for deadlifts and/or shrugs?
