If your doc is trying to get you at your high end of acceptable, then you'll be feeling pretty good. I think that the 1-1.5 IU/day amount is fine for anti-aging purposes.
I do agree that many take in the 3-4 IU/day range for more fat loss, etc., but that's an expensive way to go. It's cheaper to clean up the diet, IMO.
I'm guessing your test is 200mg/ml so that 0.4cc puts you at about 80mg/wk. If that amount puts you at the high end of normal, that's great, but if not, you should push your doc to get you up to 100-125mg/wk. Also, I'm assuming you're using a slow acting ester like test testosterone cypionate or testosterone enanthate, and pinning once per week?
Lastly, has your doc ever spoken to you about HCG - human chorionic gonadotropin - ? It's relatively common for those doing 1 shot of test per week. If you're pinning on Sun, for example, some people do 250IU of HCG on Fri and Sat, respectively, to keep their levels relatively consistent throughout.
