happens all the time, thing is, these scenes are practiced many times off the set so that the actors become comfortable enough to be able to one take it in front of the camera. There are some actors who have admittedly taped their guy down or worn specialty boxers to prevent said boner. Most directors I've been around have always told the actors they preferred them to get into the moment no matter what... which will generally mean get a boner we'll cut it out later in editing...
