most of the time a 2/0/2 on most everything, but sometimes ill do my squat with a 4/2/2 for shits and giggles
 
101, 202, 212, 303, 313, 414, 10-0-5, 10-0-10.

It depends on a number of factors which I use for any given exercise at any given time. I generally use a different cadence for each exercise, but the vast majority of my work I perform at 212.
 
Go as fast as you can while maintaining textbook form. Training slow will make you bigger but not strong, you need to train fast to become strong. I have increased my bench over 100 lbs in the last yr based on this cadence.
 
I use explosive reps. I try to explode against the weight, but then lower it under control.

I dont counter other than that.
