Do we move up in weight when say 3 sets are performed of your heaviest or when you can reach say 6 reps

brother, there are so many ways to train... there are different stages that will determine when to move up in weight... there's hypertrophy, power, etc.. so thats way too vague to attempt to answer...

If the program was do Y weight for 3 x 6 reps then yes, you'd up the weight when you can do all three sets of 6 reps
 
If you can do 3 sets of 6 reps with a given weight - you then either want to increase the weight and try to do 6 reps with the new heavier weight or keep the weight the same and try to do 3 sets of 7 reps
