how often do you change up the exercies in your routine...

I generally keep the basics in, but my routine changes week to week depending upon how my body feels. I usually get psyched up before I go to the gym and visualize part of my workout. I generally have an exercise or two in mind when I leave and fill in the rest with others. There are so many exercises for each bodypart that you can constantly change each time you hit the gym.
 
I typically never have changed it. Whenever I have lifted consistently I have always made good gains. Typically, work or school is what interferes. I'm of the school of thought that if your making good gains and continue to make good gains why mess around with it.
 
Every 6-8 weeks minimum.

Every 4 weeks is better.

You don't have to shake it up completely. Today all I did was put 2 sets of 10 rep weighted dips at the begining of my triceps workout... before my 5x5 skull crushers... instead of after.

Sometimes you change the weight... sometimes the rep... sometimes the order... sometimes the grip... and sometimes you make it whole new exercises.
