I say straight up! I'm a big fan of straight up honesty, when it comes to something like this. I believe in honesty, I also appreciate the possible sensitive nature of the topic though, so, for me, I would use some softening statements to bring it up. But, I definitely would bring it up one way or another.

Maybe you could just do something like tell her that if the u.n. inspectors smelled her breath they might take her away as a biological nerve agent so she should really brush, floss and gargle on a regular basis and if that doesn' t help go to the doctor or something.
