When do you start looking into diet drugs in the case of an obese client? Over the first year, her net loss on the scale amounts to about 7#. She has strength gains and muscle development, but it's kinda hard doing a pinch test on this gal at 235#. She is very reliable: attends regularly and follows her training routine. Not diabetic...yet, but does have a family history of heart trouble. Her progress seems too slow to me.

Are you sure there's no closet eating here? There's no way someone only loses 7 over a year of training & cardio & diet.

Ditto what Snow said. If she's a closet binger then the fat-burners are not gonna help her. Something like Xenical would be better in that situation. If she has a family history of heart probs I would not advise her to take any stimulants (such as ECA). Leave it to her Doc to check out her risk profile and prescribe the diet drugs.

You should also advise her to get either and underwater weight or DSX scan to monitor changes in body comp. Maybe she really HAS lost 30 lbs of fat and and gained 23 lbs of muscle. It would still be hard to see on someone that large.

yeah, there are diet issues, but not what you would typically expect. She doesn't eat enough and what she does eat is poorly balanced. ...ie, breakfast is often missed or she has two slices of buttered toast (maybe with a little peanut butter as well). Almost always too high in breads and fats. When I had her journal for a couple of days, I came to the conclusion that she comsumes around 600 - 1000 calories per day. Now I do spend a lot of time with this gal and although I cannot account for all of her time, I do see much of it. Maybe I need to get her into Weight Watchers.

Sounds like she's so afraid of putting on more weight that she's not only not eating right but eating way too little to add some muscle to help shed the fat. Try to drive it home to her about a proper "diet" (not as in Weight Watchers, etc but as in "nutritional intake".) Her thoughts on food intake need to be reprogrammed. Of course in the end it's all up to HER.

Put her on that Survivor island after the show is over and then go back after six months.... Bet she will have lost a few pounds by then.

Ok, I know, smart ass answer....

One thing you may need to do is to get a complete blood work up to check for any abnormality. For all you know her thyroid as shut down.

I don't now JL...anyone can journal minimal food for 2-3 days...I know...I used to eat nothing for 2-3 days and then binge for 3.

True there can be imbalances in the body but really those are rare.

WW is where I started to learn how to eat "like a normal person". Its a good start if she goes there.

Maybe shes one of those "It doesnt really count if I only take 2 bites" eaters. 2 bites of cake, 2 bites of a hamburger, 2 bites of a doughnut, etc... They pretend as if it never came near their mouths.
Ive met alot of them, and it all adds up. WE know this, but they never seem to get it.
