If you don't have an existing insurance policy forget about it, it would be considered a pre-existing condition and they won't touch you.If you do have a policy check the details of it, and try and find an ambigous rule you can exploit. The trick is to present the surgery as being "medically nessessary" get a doc to certify that the scar tissue is infected, or causing dicomfort
or possibly even effecting your mental health-effecting your self esteem, confidence, causing depression etc.So once you have the medical backup and refer to rule x in the policy they have to consider it. I am a claims assessor and believe me i've seen this work.Good luck bro.

1)Pour gasoline on the area in question.
2)Light a match.
3)Touch match to said area.
4)Inform insurance company that you have been burned in a fire and need reconstructive surgery.

If your name is glenbenton,
1) Cut into tits with a pocketknife
2) Inform police you were drugged and woke up with your tits removed.
(there is a large black market for human organs)
3) Call insurance and request reconstructive surgery.

if you can prove that you are sufering at an emotional level from your scar(you need a psychologist review to prove that) you may be able to get your insurance to pay for it...
