I have a little MS Excel spreadsheet that I keep track of what weights I'm putting up in certain lifts. I really only do it for the compound lifts and a couple other ones but before I did this I would have a hard time remembering what I did the previous week and it would hinder my progress. It's almost depressing to see yourself gain a single rep in a month but hey it's progress and better than nothing.
I am considering documenting what I eat but I dunno how long I could do that for.

I carry around a little notebook, I see people trying to get things back out of their grey matter looking all confused.

Seriously, I do keep a notebook and write down every rep and weight every exercise and every set. I can see exactly what I did last time, and improve on it every time to the gym. I see improvement, even if its 5lbs or a few more reps evertime that way.

I tried to keep track of what I eat, but its hard to know exactly how much fat, cals, protien, etc is in everything, so I felt it was a waste of time.

I kept track for a couple of months before my surgery so that now that I am back I can determine how much I've lost...

Never used to, I might have to make a habit of it now, though.

I carry a backpack in the gym with my PC in it and a 50 yard cord. I plug it in wherever I'm at in the gym. I print out a full stat sheet of my workout and hand it out to everyone in the gym.

I write it down because I can't remember anything. I also put in stuff like rest periods, tempo, etc.

Last year I was lifting with a teamate and made him get a notebook too. I would often see him scribbling in the thing, but he never seemed to remenber the weight he did the week before. One day he left the notebook in the gym, and I opened it up. Neatly graphed in spreadsheet fashion was the following:

Girls name:
Hair color:
first impression of (in 1 word, ex: scrumptous):
What they were doing (i.e., using thigh abducter for 30min straight):
What I said to them:
availability:
overall desirability:
phone #?:

Sorta weirded me out. In jest I told him he should make flashcards with each girl's pic on one side, and name and info on the other. his response "Wow! That's a great idea!!!"

I used to go by grey matter, but then I started doing lifts that were wiping me out and I couldn't remember my locker combo let alone what lifts I had done. seeing stars and stuff.
then I had a notebook but thought it was a pain in the ass.
now I have an Excel spreadsheet and I print it out, then use a clipboard to take notes, and then feed it back into the spreadsheet. that way I can chart results and see plateaus over time and such.
when I get "free time" I'm going to write a program for it all on my Visor (like a palm) - but free time is hard to come by these days.

I keep a notebook with me to write down every exercise, weight, and number of reps. Gives me something to shoot for next time I train that body part -- either try to increase by at least one rep or increase the weight if I hit prescribed number of reps on the previous workout. Works pretty well for me.

A small notebook. It forces me to try and increase my weights, and I can actually see my progress when I check back a few weeks or whatever. It can be very motivating.

Also, it helps me to change my workouts so I'm not doing the same ones week after week. It forces me to do, say incline bench instead of flat because I did flat last week. Or change from dumbbells to barbells.

I do write down everything, but not always after every set. I try to make sure it's written down before I change exercises.

I carry my planner w/ me. I keep up w/ my workouts every time. A few people at the gym just stare at me when I jot things down in my planner. I guess some people think it's silly, but they can't look back and figure why they did/didn't obtain the
goal(s) they'd set for themselves. I know why whether I might have been a little sick that day, etc.

I'D BE LOST W/O MY PLANNER!!! :O

I use a note little note pad. I think it helps me a great deal. Sometimes when I'm stuck at certain weights and can't seem to break through them I'll take a week off, it helps when you have a note pad to see what you were doing or stuck at the week befor.
About 1 a yr I'll take close to a month off this helps a great deal, writing everything down.

I have a planner that I write things down in. I make a notch after each set. Kind of like a "reward" for completing the set. When I just don't know what I want to do that evening I will to look back for a workout here or there that I remember was really intense or had good variety or whatever. It's pretty motivating to see increases in weight or reps. I like it.

Nothing like checking back 2 or 3 notebooks ago and saying: "Shit! I was a weakling!" For me, no notebook = no workout.
