It can be very painful if you happen to stick it in the wrong way. You never know if it will nail a nerve or 12. It is probably about 60% likely that you will be in pain, even with an insulin needle. I have a "friend" (of course not me, this stuff is illegal) who used to shoot winnie into the inside head of his triceps and the inside head of his calves. The regular tricep shots, 1/2 cc in each - 1 time a week, did definitely show improvement. The shots were not too painful, but occasionally a nerve was hit or a blood vessel was ruptured (huge bruise). In my opinion, it was worth the trouble. The shot in the gastrocnemius was not so successful. The intense cramping that followed the shots, combined with the pain doing it, did not make it worth while for this person. In fact, there were a few times that when the needle was removed, blood would squirt out with incredible pressure. Not fun.
I guess the question "are the gains worth it?" must be answered by you. I think this kind of thing should only be done by people who are about as big as they are gonna get, and would like to improve a lagging bodypart. If your diet or workout program sucks, then this won't make any difference. You will definitely need someone else to give the shot, as you will be too busy grimacing and squeezing the shit out of whatever is near your hands. Someone else posted a response to this, and saud that they had tried to use laurabolin for this. You will have to use a water based juice to achieve any local growth from a shot. That would mean winnie or suspension (I hate suspension). Don't do this kind of crazy shit just because my "friend" had some success. Do it because you are sure that this is what you want to do.
